Ques/Help Moderator Panel Missing and more

I am unsure how I should proceed from here. If I need to do a fresh install or what. Something has clearly broke.

I am no longer seeing the following
Moderator Bar
User info with PM area



By user info and PM area, I mean this


I have myself added to moderators and give full perms. I have tried disabling all addons, still nothing.
