Bug Prefix changes are being logged twice in Moderator Actions

My prefix edits when I change to another prefix on a member post, they get logged twice in Moderator Actions. They look like this:
Thread prefix edited from 'x' 3 minutes ago
Thread prefix edited from 'x' 3 minutes ago
